Every GP on completion of training is required to undertake an annual GP appraisal. This process will lead on from your eportfolio. The current GP appraisal system is explained below. Many GPs complete their appraisal documentation electronically using the GP NHS appraisal toolkit.
Forms 1,2 and 3 must be completed before the appraisal discussion. The first two forms are brief and factual. Form 3 requires care. the GP is invited to submit documents in support of Form 3 and these will need to be assembled. Completion of the form and collecting the supporting material is teh GPs responsibility, although the GP tutor may provide advice if required.
Form 3 is prefaced with explanatory notes.
Some of the material the GP provides now will carry forward without further work to future appraisals. Your appraiser will bring two further forms to your meeting. Form 4 is the formal summary of the appraisal and should be completed during and immediately after the discussion. This is the responsibility of the Appraiser. In addition the appraiser will bring along a form to record your personal education and development objectives for the coming year.
The forms below were used by the Scheme prior to the introduction of the eportfolio. They are the same as those used by GPs in their appraisals, with minor modifications.
