These first five links below provide essential information about the programme which you must read and understand as soon as possible.

Paperwork requiring completion:

The most up to date versions of all relevant documents required for:

• Recording of Out of Hours training experience (Form OOH1)
• Confirmation of level of supervision required (Traffic Light Forms OOH3 & OOH4)

should be accessed on the Deanery website under Primary Care/Out of Hours.
